Do this FIRST!!!

• Download and fill out your Intake Form FIRST.

  • If you do not have a printer, contact me and I will send you the forms with return label. ADD $15 to payment for this.

 

• Prepare Sample: page 10 and send them WITH paperwork. May put H.N.U.S. in the same bag. Please double bag and seal. Label the outer bag with name and date of birth. Send a check for $250 with your paperwork. I will call you when it is recieved to make an appointment (via zoom or in person).
